UIST99 Registration Form

Hello, and welcome to the UIST99 registration process. In order to register, you will need to perform the following steps:

Fill out this form online.
Press the "Make Letter" button at the bottom of the page.
Print out the registration form that button created.
Fill in additional information (credit card info, signature, etc.).
Send it to the address below, either by physical mail or by fax.

After you have sent your registration materials in, you will receive confirmation by email. Physical mail confirmation will be sent only if you request it. If you don't receive email confirmation within a week, please contact the registration chair:

Cancellation policy. If you need to cancel a registration, and if we receive written notification by November 1st, then we will refund your registration, minus $50. After that, no refunds will be issued.

You must arrange accommodations directly with the Conference hotel . Reservations should be made by October 8th to receive the conference room rates. If you are planning to use the conference hotel, you need to specify that you are a ACM User Interface Software and Technology attendee (group number 203584), to get the UIST rate. Therefore, you should not use the Web site to make reservations - if you do, you won't get the UIST rate.

²To be a student volunteer, you must first talk with Anind Dey and be chosen as a student volunteer. Don't check this box unless and until you have done this!

Dining at the Grove Park Inn

The restaurants at the Grove Park Inn have a dress code after 6pm requiring collared shirts and long pants for men. The Grove Park Inn also has one formal dining room requiring suit and tie. Informal attire is fine for breakfast and lunch. There are also restaurants within driving distance of the Grove Park Inn. It may be a somewhat long walk to these restaurants.
